New form for "claiming compensation for damage caused by vaccination"
The Ministry of Health has published a new form in order to claim for compensation for damage caused by vaccination.
You can download the said document as well as an FAQ sheet at the bottom of this post. The two documents exist, for the moment, and to our knowledge, only in French!
Attention:
The completed form should be sent to the Ministry of Health:
- by e-mail to the new address indemnisation.vaccination@ms.etat.lu
- or by post to the following address:
- Ministère de la Santé
- L-2935 Luxembourg
In this case, the postal address is not that of the Pharmacovigilance department!
The form is hidden away in a menu on the Ministry of Health website. However, the function for downloading this form is lacking.
